This is to inform all concerned that the following services of the examination department of the National University are being provided online. To avail of these services, one has to apply by entering the Student Login of the Service Menu of the National University Web Site (www.nu.ac.bd). National University Certificate/Mark sheet/Duplicate Admit Card Application Form Document
All the documents to be attached online (scanned) for the services provided are mentioned in the notification: The following required documents should be attached with the online application.
1) Admit Card Correction - Application with the recommendation of the Superintendent - Photocopy of the amended registration card - Original copy of the amendment or attested photocopy - Fee 500 (five hundred) Taka (through Sonali service).
2) Duplicate Admit Card - Application form with a recommendation of the principal - GD copy of the nearest plate and lost notification of daily newspaper - Attested registration card. -Fee, 500 (five hundred) Taka (through Sonali service)
3) Provisional Certificate - Application with the recommendation of the Principal - Attested photocopy of registration card and admission card - Attested photocopy of number card - Fee, 300 (three hundred) Taka (through Sonali service) - 1993-2000 (Manual)
4) Mark Sheet - 6
5) Duplicate Certificate Mark Sheet - Application with the recommendation of the Principal - Jikti copy of the nearest police station and lost notice of Sainik newspaper - Registration capt and certified photocopy of admission card - Manual from 1993-1999 - Fee 300 (three hundred) Taka Through golden service) |
6) Transcript - You have to apply in a specific form. - One copy of passport size photo, registration card, all admission cards, certificate, number sheet, and attested photocopy of SSC examination. - Detailed information is given in the form. -Fee, 600 (seven hundred) Taka (through Sonali service)
6) Original Certificate - Application form with the recommendation of the principal - Attested photocopy of registration card and admission card - Attested photocopy of temporary certificate and number sheet. - Fee 500 (five hundred) Taka (through Sonali service)
6) Duplicate Original Certificate - Application with a recommendation of Superintendent - GD copy of the nearest police station and lost notification of daily newspaper - Attested photocopy of registration card and admission card - Attested photocopy of temporary certificate and number sheet - Fees Via Sonali service)
9) Document Verification - Attested photocopy of all the documents that will be verified, certificate, number sheet, and transcript - Film 300 (three hundred) Taka for each document (through Sonali service)
Note: Application for the original certificate is applicable only to students who have passed the examination from 1992 to 2000. The temporary certificate must be returned at the time of receipt of the original certificate.

জাতীয় বিশ্ববিদ্যালয়ের মূল সনদ উত্তোলন, মার্কশীট, দ্বীনকল সার্টিফিকেটের জন্য আবেদন করার নিয়ম নিয়ে আজকে আলোচনা করব। জাতীয় বিশ্ববিদ্যালয় থেকে সার্টিফিকেট বা নম্বরপত্র certificate / marksheet উত্তোলন করার নিয়ম নিম্নে দেয়া হল।
২) ভোটার আইডির ফটোকপি দরকার।
৩) online থেকে সনদ তোলার নির্দিষ্ট ফরম আছে সেটা পূরন করে ভালভাবে download করতে হবে আর কলেজ principal এর কাছ থেকে sign করে নিতে হবে ।
৪) তারপর আপনাকে গাজীপুর national university head office এ যেতে হবে সেখানে bank draft করতে হবে নির্দিষ্ট ফি দিয়ে।

এতদ্বারা সংশ্লিষ্ট সকলকে জানানাে যাচ্ছে যে, জাতীয় বিশ্ববিদ্যালয়ের পরীক্ষা বিভাগের নিম্নবর্ণিত সেবাসমূহ অনলাইনে প্ৰদান করা হচ্ছে। উক্ত সেবাসমূহ গ্রহণের জন্য জাতীয় বিশ্ববিদ্যালয়ের ওয়েব সাইট (www.nu.ac.bd) এর service Menu এর Student Login এ প্রবেশ করে আবেদন করতে হবে । Student Login এ Download Instruction বাটনে অথবা সরাসরি https:/services.nu.ac.bd/nu-app/download/Online-Student-Services-Instruction.pdf থেকে Download করা যাবে।
প্রদত্ত সেবাসমূহের জন্য যে সকল কাগজপত্র অনলাইনে (স্ক্যান) কপি সংযুক্ত করতে হবে তা বিজ্ঞপ্তিতে উল্লেখ করা হলো: নিম্নে বর্ণিত প্ৰয়ােজনীয় কাগজপত্র অনলাইনে আবেদনের সাথে সংযুক্ত করতে হবে।
১) প্রবেশপত্র সংশোধন (Admit Card Correction) -অধঃক্ষের সুপারিশসহ আবেদনপত্র – সংশোধিত ৰেজিষ্ট্রেশন কার্ডের ফটােকপি – সংশোধনের মূলকপি অথবা সত্যায়িত ফটােকপি – ফি ৫০০ (পাঁচশত) টাকা (সোনালী সেবার মাধ্যমে) |
২) দ্বি নকল প্রবেশপত্ৰ(Duplicate Admit Card) -অধ্যাক্ষের সুপারিশসহ আবেদনপত্র – নিকটস্থ থালার জিডি কপি ও দৈনিক পত্রিকার হারালো বিজ্ঞপ্তি – সত্যায়িত রেজিট্রেশন কার্ড | -ফি, ৫০০ (পাঁচশত) টাকা (সোনালী সেবার মাধ্যমে)
৩) সাময়িক সনদ উত্তোলন(Provisional Certificate) -অধ্যাক্ষের সুপারিশসহ আবেদনপত্র – রেজিষ্ট্রেশন কার্ড ও প্রবেশপত্রের সত্যায়িত ফটােকপি – নম্বরপত্রের সত্যায়িত ফটােকপি -ফি, ৩০০ (তিন শত) টাকা (সোনালী সেবার মাধ্যমে) – ১৯৯৩-২০০০ সাল পর্যন্ত (ম্যানুৱেল)

৫) দ্বি নকল সনদ/ নম্বরপত্র(Duplicate Certificate Mark Sheet) -অধ্যাক্ষের সুপারিশসহ আবেদনপত্র – নিকটস্থ থানার জিক্তি কপি ও সৈনিক পত্রিকার হারানো বিজ্ঞপ্তি – রেজিষ্ট্রেশন কাপ্ত ও প্রবেশপত্রের সন্ত্যায়িত ফটােকপি – ১৯৯৩-১৯৯৯ সাল পর্যন্ত মানুয়েল – ফিঃ ৩০০ (তিন শত) টাকা (সোনালী সেবার মাধ্যমে) |
৬)ট্রান্সক্রিপ্ট(Transcript) – নির্দিষ্ট ফরমে আবেদন করতে হবে । – পাসপোর্ট সাইজের এক কপি ছবি, রেজিষ্ট্রেশন কার্ড, সকল প্রবেশপত্র, সনদ, নম্বর পত্র ও এসএসসি পরীক্ষার সত্যায়িত ফটোকপি । – বিস্তারিত তথ্য ফরমে দেয়া আছে । -ফি, ৭০০ (সাত শত) টাকা (সোনালী সেবার মাধ্যমে)
৭) মূল সনদ(Original Certificate) -অধ্যাক্ষের সুপারিশসহ আবেদনপত্র – রেজিষ্ট্রেশন কার্ড ও প্রবেশপত্রের সত্যায়িত ফটােকপি – সাময়িক সনদ ও নম্বরপত্রের সত্যয়িত ফটােকপি | -ফি ৫০০ (পাঁচশত) টাকা (সোনালী সেবার মাধ্যমে)
৮) দ্বি-নকল মূল সনদ(Duplicate Original Certificate) -অধক্ষের সুপারিশসহ আবেদনপত্র – – নিকটস্থ থানার জিডি কপি ও দৈনিক পত্রিকার হারানো বিজ্ঞপ্তি – রেজিষ্ট্রেশন কার্ড ও প্রবেশপত্রের সত্যায়িত ফটােকপি – সাময়িক সনদ ও নম্বরপত্রের সত্যায়িত ফটােকপি -ফি, ৫০০ (পাঁচশত) টাকা (সোনালী সেবার মাধ্যমে)
৯) ডকুমেন্ট যাচাই (Document Verification) – যে সমস্থ ডকুমেন্ট যাচাই করবেন সনদ, নম্বরপত্র ও ট্রান্সক্রিপ্টের সত্যায়িত ফটোকপি – প্রতিটি ডকুমেন্টের জন্য ফিল্ম ৩০০ (তিন শত) টাকা (সোনালী সেবার মাধ্যমে)
বিঃদ্রঃ মূল সনদের আবেদন শুধুমাত্র ১৯৯২ থেকে ২০০০ সালের পরীক্ষায় উত্তীর্ণ শিক্ষার্থীদের ক্ষেত্রে প্রযোজ্য। মূল সনদ গ্ৰহণকালে সাময়িক সনদটি ফেরৎ দিতে হবে।
